I am thinking about keeping the elbow pads low and raising up the handles about eye-high.

Talk me out of it!

Go for it.  I recall hearing an episode of tri-talk (the podcast) where he reported a wind tunnel study.  I can't remember the details (so beware...) but the short version is that they did wind tunnel tests in three positions -- flat, 'medium', and 'angled up considerably' (maybe 'eye-high'?) The best position was the last.  But medium was worst.  There was a catch somewhere but I can't remember what it was.  Anybody recall?

are we talking about "praying mantis" here?

Some of the elder guys can say more on it. As I remember it caused some precarious handling issues though.

There is a well known, highly regarded, world ranked triathlete who does this.  (With 4 beers in me, I cannot remember his name).

It just hurts my shoulders to go forward with the aeros.

You can set up like TJ and use nut cups for your elbows

Where does it hurt in your shoulders and how long into a ride in aero does it take before it hurts?

I'd want to master the aerobars before I tackled TJ's position. That looks like a wreck waiting to happen.

WTF, I already wreck a couple of times a month.  I might as well be comfortable before I wreck.

Wow.  That's fast.

The reason I asked is the shoulder pain I was getting this summer turned out to be aero bar pads that were too narrow.  It was more musculature and would occur after about 45 minutes to an hour in aero.  The only scapular pain that I had in the past was from too great of a reach.  Not sure if tilting your bars to eye-level would help.  What does the fit guy at your LBS say?

Would that be a "needle in the butt" set-up?

There's two problems the way I see it... first, the line between aerodynamic excellence and disaster is extremely fine with this set up... we're talking hand position up/down within 1/4"... too low and you will not shield you body from the wind, and too high, you'll create a perfect windtunnel for the airflow to hit your chest... This position pretty much has to be set up in a windtunnel to be efficient...

Second problem is the instability... unless you're an amazing bike handler (and by your own admission, you have crashed a few times already this month...), this is downright dangerous...

All in all, my recommendation would be to stay away from this, but as everything else, you choice.

I am looking for any advantage I can get to move up from FOMOP to BOFOP.  A huge difference!

Why not spend a few hundred bucks and get someone to fit you? Getting fit is not a one and done thing, sometimes you have to play with it and have several fits to get it right and then as you get more aggressive due to fitness, you have your bike fit checked again.

I have been fit and had several changes to my position, I also get tune ups every once in a while to make sure I am dailed in.

I changed aero bars one time and started getting pains in the bone part of my fore arm, I went back to my guy and we tried everything, changed stems, pads, raised pads, narrower pad placement etc. We finally got it dailed in after 2 hours on the trainer in the bike store. I then went back a few times for little changes to get it dailed in. I feel like I am laying on the couch with I am in aero and prefer to ride that way over sitting up; that is comfortable.

Instead of trying something radical and risky for handling, if you are serious about this sport then you will be in the aero position for long periods. Spend the money and get someone who knows what they are doing and get it done right.

It will make all the difference in the world.

I will be doing this with a fitter.  It will be my fourth fit in a year, for just my TT bike.  I have also been fit for my mtn and roadie.

to his defense, he's riding with his elbows in jock straps, so the position isn't all THAT weird

But, he makes money doing tri's and I don't so...

tell him all of your issues and he should be able to get it dailed in for you to make it less painfull. Sometimes that means not being as agressive in the set up...

doesnt matter how aggresive the position is and how little the frontal position is, if you cannot stay in it then the position is not going to help you any.

Dont buy into the whole lower is better BS... get into a position that is right for you, not what looks good on other riders and pros.

I think this is a big issue for new riders, they want to get into this super low aggressive position to look cool and aero but then end up sitting up and riding on the bars because the position is too agressive and it hurts them.

not saying this is you Pga Mike but stating for other people... Sometimes being less aggresive works also. I would rather have a less aggresive position and be able to be comfortable in the position than a super low position that you cant stay in and have to sit up to ride...

Dont let the fitter tell you lower is better, explore all positions.

HE is also a engineer and spent time in the wind tunnel confirming this position is best for HIM....

yeah i am going to have to say i would be careful with this.

dropping the pads and raising the bar ends is going to do a few things. you are adding a LOT more drop, steering is going to get a LOT harder.

and it may be slower. (this position is not faster foreveryone).
