Surgical decisions are usually based on 1-if you have a slip or not and 2-the chronicity of the injury.  If its acute and no slip I would encourage you to try conservative care with the brace and this usually works.

Its hard to know when it occurred then.  If you were diagnosed on bone scan and it hot then it will probably heal.  As far as the slip (Or spondylolisthesis)  We grade them 1-2-3-4....1 is 25% or less slip 2 50% 3 75% and 4 is all the way off.  The bigger the slip the more the need for surgery, also if it is stable or unstable.  Usually they will have you do flexion/extension views of your lumbar spine.  I would encourage you to see an orthopedic spine specialist.  Are you having and trouble with leg pain or is it all back pain?  These are not uncommon in athletes esp gymnasts and linebackers

Thanks! So far it's just back pain, no leg pain. It's a 1 I think. 

I think its well worth trying conservative care first.  You will miss a few months of training but probably avoid spine surgery and they will most likely let you swim and bike during your recovery

There is hope!  I have a grade 2 L5/S1 spondylolisthesis.  Never had any pain or symptoms until I started tri training 3 yrs ago (this is after having 2 kids, doing 12 marathons and several other road races, strange weight lifting activities, etc).  I've always managed by maintaining excellent lower abdominal strength, good body mechanics, and, did I mention good ab/core strength?!  Really my only discomfort is as a result of an unstable SIJ (sacroiliac joint. as a result of the spondy) which requires manual techniques (by a chiropractor, I have to admit b/c I'm a physical therapist ).  All is well and it hasn't slowed me down much at all.  Please understand, each human body responds to different things differently.  but I share my story to let you know that there is hope in conservative management. 

...did i mention the importance of good ab/core strength and excellent body mechanics??

I just read your post, I have grade 2 L5/S1 spondylolisthesis just like you. I am 57 yrs old, 6'-2" and weigh 200lbs, trying to cut my weight down. What are the exercises you do to strengthen your core? I traditionally run more heel to toe with a slower pace of about 165. Would running a faster pace and more forward on the foot help?

I also have spondy in L4-5/L5-S1 and my L5-S1 disk is nearly gone, plus a few other abnormalities in that area.  I was in constant pain for 2.5 years without a diagnosis, mostly because I ended up having 2 hip surgeries (torn labrum, which I then re-tore 6 months later).  When I was about 4 months out from my 2nd surgery, I decided to look into the back pain again.  Went to an ortho surgeon, who told me I needed a fusion.  Being 45, that sounded pretty scary.  I found a chiropractor who specializes in rehab medicine.  He's been doing acupuncture, stretching, and manual manipulation on me as well as having me do some core exercises 6 days a week.  As a result of seeing him, I've gone from not being able to do pretty much anything in mid-January to now training for my first tri and doing really well.  Here's a link to some exercise videos on his site:  http://www.activachiropractic.com/Exercise.html.  Good luck!
