Subject: Help diagnose my hip pain please?
Background: I've been a middle of packer triathlete for 5 years now. Mostly do sprints, 1 oly under the belt. I've been getting a lot of pain in on side of the hip for months now. I have had to stop training. I've been to see a Chiro who specializes in Active Release Technique and it has helped a little but now a lot. The pain is in the upper thigh/ Groin area. It hurts to lay on my good side with bad hip up in the air. There is a little pain in the upper IT band/glute area at times. The upper thigh/groin is where it really hurts the most. It's better in the morning but starts hurting as I move around more during the day. I've been thinking possibly hip bursitis or maybe piriformis syndrome? Has anyone had problems with these? Running is out now, biking doesn't hurt bad but I have quit that too, swimming is no problem as I tend not to kick a lot anyway. Please help I'm going nuts sitting around. Thanks.

I've been limited to swimming only for the last three months and just finished 2 months on crutches because of a femoral neck stress fracture that started out with symptoms like yours.

I agree with the comments to see a sports MD who can evaluate and determine if it's something that can be treated through PT, or if you need to seek further help from an orthopedist or neurosurgeon.

I'm a doc, but I can't make a diagnosis without physically seeing and touching a patient. I could try, but it would be irresponsible. You deserve the same type of evlauation that any other patient who walked into a sports med office woudl get...You need a hands on physical exam where the physican can push in all the right spots, do muscle testing, joint testing, etc.

When you have a correct diagnosis, you can then pursue the treatment of your issue in order to get back to training.

If you aren't already, try stretching your iliopsoas or 'psoas' -- google it, there's a bunch of illustrations for stretching the hip flexor out there.  This body part has bugging me recently from running, especially uphill.

The stretching picture in the article below shows her front foot behind the knee, but I recall one site said to keep that foot out in front of the knee.  Go easy, see if it helps.  Hope things get better for you.  And like the other posters said, if you can't solve it with some stretching, a visit to the doc isn't a bad idea.

If the pain is in the upper thigh/groin region it is most likely not piriformis syndrome.  It could be several things such as a hip flexor strain or you could have a rotated pelvis.  Your best bet is to see a Doctor and get a referral to PT.Smile  Good Luck!

Thanks for all of the advice. As of yesterday I pretty much decided to see a specialist. My family doc (who is a endurance sports type) has thought I should see a PT, but after not much inprovement with ART I am afraid of wasting more time and more $. I figure the specialist hopefully will be able to diagnose properly to get me on the recovery track. I really want to get active again :-).

Do you always run on the same side of the road? Like always against traffic?

I developed hip brucitus from doing just that. The uneven grade of the road throws your hips out of line.

Its not your piriformis...thats in the back not in your groin. Could be something simple like a bursitis tendonitis but be safe.  Go to the Dr, get and xray.  If nothing shows up then get a bone scan or an MRI.  Better to get a negative test than to have something bad like a stress fracture.

I cannot diagnose your pain, but I definitely agree that a sports med visit is in order.  I have one friend with hip/groin pain who ran in constant pain and was later diagnosed with a laberal tear.  I had another with groin pain who also ran on it and had a stress fracture.  Both were out of their sport for a long time.  You would not want that to be you.

It could be it is something simple to correct with rest or PT, but better to be safe than sorry.

I am definately going to get check out, but sue, I was leaning towards bursitis and I run on crowned country roads, and a sloped track at our ymca, so your response gives me something else to wonder about. Thanks for all the help everyone, this site is great.
