Subject: Big Toe Joint Pain
The joint of my right big toe started to swell about two months ago.  After about 4 days of swelling, it started to bruise.  All this time it was very painful to walk on, let alone run, so I went cycling.  Turns out that wasn't so good for it either, as it then hurt even without putting weight on it.  After 9 days of swelling, I went to see the doctor, and he predicted gout.  He took an x-ray, which looked normal and did a uric acid test.  The test came back negative, so no gout (thank goodness).  The swelling started to go down after 10 days.  The doctor told me to stay off of it for three weeks, which I did with the exception of walking .5 miles to class and back every day.  I finally went running in the fourth week-just a short, easy 3 mile jog.  I did another hard fast 1.5 mile run a couple days later.  A week after that, the pain came back with much more mild swelling and a slight bruise.  I called the doc, he told me to stay off of it and that's all he could do for me.  I've been off of it for 2.5 weeks, and the pain is not getting any better.  I NEED to be running, and I need the stress relief.  Is there anything I can do to get rid of this pain?  Has anyone experienced this?  I know I need to get another doctor to look at it, but I'm in a small town, and haven't found anybody who really cares at this point, although I do know of a good physical therapist.  Are there any suggestions out there?

Hi!  I have some pain (with some swelling at times) at the joint at the bottom of the big toe.  I noticed it after a period of treadmill running.  (I think it was also related to my shoes at the time...)  My podiatrist is going to correct some of it with orthodics - but also recommended not wearing heels (which causes discomfort).

In your case it's much more severe than mine - but what about aqua jogging?  That way you can still run but you won't be aggravating it... I know - not fun - but at least it will have a chance to heal.

Ooooh...yuck.  I've actually never tried aqua-jogging, but I guess it can't hurt.  However, I'm still very concerned about the actual problem.  I wish I could see an orthopedist, but because my primary physician won't take me seriously, I can't get him to recommend an appointment with a specialist.  He even refused to recommend physical therapy.  I guess I'm just going to have to travel the distance to see a better physician.

I'd do whatever it takes to find another physician.  I have swelling around my big toe joint--it's due to a bunionectomy gone bad and I could have it re-done, but don't want to go through surgery again so I am doing PT.  They have iontophoresis and anodyne which work wonders in reducing the inflammation, and therefore reducing the pain.  It sounds like this is interfering with your livelihood and that makes it seem worth it to find a better doctor who can get you some physical therapy--or whatever it is you need for this pain.

Definitely need to see an Orthopedist, preferably one who specializes in the foot and ankle.  Serum uric acid level really doesn't corelate well with Gout, ie, many people with high levels don't have gouty arthritis and many people with Gout have normal levels.  There are many other inflamatory conditions of that joint as well.  Did you doc have you on any anti-inflamatories, like Indocin??
