If they have a registry, get the most practical thing off of that.
For our first child, we ended up having 1 1/2 carts of returns from gifts.

We ended up keeping a lot of stuff that we did not like... because those were the ones that had no gift receipts not could we identify a store that had them in stock to allow a return.

The stuff we did like had givers conscientious enough to give a gift receipt.

Unless they ask for it, nothing that requires batteries.

My mom SWEARS the baby bag is the best thing since sliced bread as have every new mom I have ever given one too.  Its like a onesy but instead of legs its just a bag.  Super easy to get on and off for diaper changing etc.  I also usually get itty bitty things as most people buy something the baby will "grow into" and then the kid doesn't have anything to wear the first few weeks.

The best shower gift I received was a fantastic bottle of rum.
The second best was the glass to drink it in.

We received a bunch of blankets and typical baby stuff. If you want something special, get them a photo pack done by an expert. Give them a memory they can keep forever.

Might I highly recommend against Baby Bjorn's and instead look at a sling. Baby Bjorn's put entirely too much pressure on a baby's crotch and it only gets worse as they get bigger. Plus, if you're weirdos like me and my lady, having your baby facing away from your body, as opposed to keeping it close to your heart in a sling, is bad attachment parenting.

How about a gift certificate to use for a membership here at BT?

One of the gifts that Mrs Chad and I got that was really helpful was a whole bunch of stuff we'd not thought of as first time parents.  Infant tylenol, gripe water, etc.  Things that an experienced parent thought of, that first timers wouldn't think of.  Sure it wasn't cute or "awww" inspiring, but it was way more useful than the mobiles and such that we got.

Brad, I fixed that for you.

No really, there will be TONS of stuff for the baby through other people and the cutsie outfits will soon be grown out of.  Do something for the parents like others said.  First get some beer for the dad.  What is the price range?

I'd bet she already has a lot of "stuff."  How about a gift card for a restaurant that delivers or a cleaning service?  A homemade meal or offering to do a chore at her house would be very thoughtful as well - my lawn was so overgrown after having my son, I would have loved it if a friend/neighbor offered to mow it for me!
