I wear a 13 2E/4E shoe normally and I found SPECIALIZED SPORT MTB SHOE 6116 in a 14 works great!

You might also give Diadora a try -- they run pretty wide.

SIDI does make shoes in Mega width ... I'll also add that Mike and I have the widest feet (for our foot length) on BT.

I cycle in SIDI Mega, myself. FWIW my feet are about a men's 10 in 6E width. Here's a pic:

As others have said, Sidi has an amazing range of sizes/widths - and if you have an unusual size foot they're a decent bet.  I'm a size 15B so a slightly different challenge than what you have, but Sidi has separate shoes for narrow/normal/wide widths - so I'd give them a look.  They were the best fitting cycle shoes I've found for my own variety of freakish feet.

They're not the cheapest shoes, but excellent quality and very comfortable in my experience.

I wish that my feet looked that healthy.  Unfortunately, my toes are absotively squished together from years of cramming them into 9.5 EEE shoes.  :0

I found a pair of Northwaves that have a nice roomy toebox that worked great for me.  I found a lot of the other cycling shoes if they were the right length, pinched my toes together, so I either had to buy the wrong size to get the right width, or deal with the toe pain.  I opted for neither, and am very happy with the Northwaves.

Go to a LBS and try one several pairs in different brands and make notes about what sizes work best.  Just try not to make them cry when you leave without paying $200 for a pair of shoes.  You will find much better deals for cycling shoes online.

I have Fred Flintstone feet and finally found Shimano R220.  They fit my Double E's very nicely...
