Subject: red urine?
I went for a long training run today, it was a significant increase in distance. approx 4 hours later I realized the red urine issue... no pain, just a color change. Is this a normal thing after a hard workout that the body is not used to to have a change in bodily functions?
Should I be concerned and go to the hospital or just drink more clear fluids and not worry about it?

Hi TJ--if you're not having pain it's probably okay (all sort of weird things can happen when you do weird things like run/bike/swim crazy distances!)  Obv. if it persists, gets worse, causes pain you should seek medical advice.

Both my friend and I had this after a marathon ... he didn't have pain either, although I was in a lot of it (but I had started the marathon sick and had major other issues going on).  In both our cases it turned out that our kidneys had gotten a little bruised from all the bouncing and we were expelling a bit of blood in the urine.  No biggie and it went away in a couple of days.

Definitely work with the hydration issue (never hurts to replenish anyway) and contact spokes!

99% sure that hydration is the issue if you're not having any speciic urinary symptoms like burning/frequency/flank pain/urgency/fever...That being said, if your urine doesn't clear after well hydrating yourself and/or you develop any of the above symptoms PLEASE be evaluated at the local ER. BTW, kidney stones like to develop in a dehydrated person--seems a favorable condition for them but I'm not certain exactly the mechanism of why

Hydrate and if you are still bleeding tomarrow you should see your PMD or go to an ED to be checked out.

More likely diagnosis than hematuria (blood in the urine), is myoglobinuria - urine discolored by the excretion of muscle breakdown products. This can indeed reflect hydration problems, and rehydration is key to avoid the toxic effects of myoglobins on the kidney. If you rehydrate and the problem resolves, probably no big deal. If the discolored urine persists, urinalysis and a visit to the urologist are necessary - could be anything fron a stone to infection to autoimmune disease to who knows what. Kidney failure is not a pleasant problem to deal with.
Just my two cents, hope that helps.

True - I had some killer cramping after a half. My bride was working the E.R. so I stopped to see her and drop a few liters of saline. Had tea colored urine. She drew some labs and although my major 'lytes were fine, calcium was out of wack, CPK and myoglobin levels were way up. Two liters later life was back to normal...
Sorry, forgot a little educational material on rhabdomyolysis: http://www.sportsinjurybulletin.com/archive/1039-rhabdomyolysis.htm
Hope that helps!

For completeness sake: A urologist friend suggests also a diagnosis of sports hematuria - supposedly common in marathoners. He sez it is due to collapsed bladder walls (no pee in there if you're dry) rubbing on each other during vigorous exercise. Same recommendations - hydration and medical eval if not spontaneously resolved.

If it doesn't clear quickly a visit to the ER is in order, as the effects of myoglobinuria/rhabdomylysis can be kidney failure. IV fluids designed to alkalinize the urine, while maintaining a high urine output is the treatment...that' can't be easily done at a doctors office, but the ER is ready and waiting to treat just such things.

Things seem to finally have gotten back to the way they are supposed to be, as of yesterday afternoon. I have been chugging lots of water, and that has helped.

It's kinda scarey to look down and notice you are peeing red.

Thanks everyone for the advice.

This threads reminds me of an idea I once had for a contest:

Post an exercise-related symptom and see which BT'er can come up with the most frightening possible diagnosis. Two categories: AG (lay folk) and Pro (MD's).

A young woman went to her doctor complaining of pain. "Where are you hurting?" asked the doctor. "You have to help me, I hurt all over", said the woman. "What do you mean, all over?" asked the doctor, "be a little more specific."
The woman touched her right knee with her index finger and yelled, "Ow, that hurts." Then she touched her left cheek and again yelled, "Ouch! That hurts, too." Then she touched her right earlobe, "Ow, even THAT hurts", she cried.
The doctor looked at her thoughtfully for a moment and asked, "Are you a natural blonde?" "Why, yes," she said. "I thought so," said the doctor, "You have a broken finger."

I vote for the empty bladder / bladder walls rubbing theory.

Yes, several times in my career, more so recently, I have been caught with coffee colored urine. Frightening when I don't expect it. Has always cleared right up.

If you google and try to read about the causes you'll find a lot of hooey theories as to what causes it.

Anyhow, I make certain if on a long run and take a leak, that I also am hydrating ahead of myself so as to get the bladder with something in it to avoid the problem. Maybe I am too into this running thing. Jeesh.

Best of luck --

I had similar issue last June...I did a tri Sunday and another on Thursday. My urine looked like apple cider. Went to the Doc, had lab test done...lots of blood in my urine. The concern was what another poster suggested which Adventurebear elaborated on.

I had follow up lab tests done a week later. Then scheduled an office viist with labs the day after my next Oly race. Those labs checked out fine.

It scared me a lot. The thought was I had a small kidney stone perhaps as the exercise induced illness didn't really fit as I did only two sprints not a lot of time.

It cleared up in a couple of days, and I have had no futher issues. I made sure I drank plenty at my tri last weekend, but it was a sprint - don't think I'd see those issues.

Hi Guys,

I would not want anyone to be alarmed, or suggest that their situation was necessarily the same as what I encountered this year, but sometimes blood in the urine is the symptom of a real problem.

After an 8 mile run 2 months ago, I had blood in the urine. It cleared up by the next day (at least was not visible to the eye). I went to the dr, who sent me for an Ultrasound (entire abdominal region). They found an issue with my bladder, which turned out to be the source of the 1 time bleeding episode that I had noticed. The issue was bladder cancer and I had surgery to remove "Squiggly" (my name for the nasty guy that was growing there).

So Squiggly is gone and the lesson leaned is: Don't be afraid to ask for some easy testing to ensure that there is not a bigger problem. Chances are, it will be a minor issue (bladder walls rubbing, minor kidney issues that are temporary, or other).

I just would not necessarily assume a one time occurrence of blood in the urine is a non-issue.

Anyhow, hope that helps.

Danny (and Squiggly)
